Peru Health Ministry provides guidance on filling optical forms for Serums 2026-I evaluation

The Ministry of Health (Minsa) announced on Apr. 7 that it is providing instructions for properly completing the optical form to be used in the evaluation for the Rural and Urban Marginal Health Service (Serums) 2026-I, which will take place nationwide on Sunday, April 19.

The guidance aims to ensure that participants accurately fill out their identification and answer sections, as errors could lead to exclusion from grading. The ministry said the correct use of this form is essential for maintaining the integrity and reliability of exam results.

According to Minsa, participants must write their assigned classroom number using three digits and shade in corresponding circles with a 2B pencil. They are also required to enter their eight-digit National Identity Document (DNI) number by shading one circle per digit in each column. Foreigners with an Alien Registration Card should only record the last eight digits from the right side of their card number.

Participants are instructed to clearly print their paternal surname, maternal surname, and full given names within designated boxes without exceeding boundaries. The response sheet consists of 100 questions with four possible answers each; only one answer per question should be marked. If two circles are filled for a single question—even if one is correct—the response will be considered invalid due to multiple markings.

Minsa reminds examinees that proper completion of both identification and answer sections is solely their responsibility. Any mistakes may result in disqualification from scoring. All health professionals participating must bring a 2B pencil and carefully review all information before submitting forms for processing by optical reader technology.
